An employee was attempting to clean an ink spill on a belt using a rag when a second employee jogged the belt and the first employee's rag became caught in the belt. As the first employee attempted to grab the rag, the employee's hand and fingers were caught in the belt, resulting in broken fingers and lacerations. The belt was not locked/tagged out at the time of the incident.

Employee was driving a scissor lift through a large doorway. When the employee released the controls, the scissor lift jerked backward and caused him to hit his head on the top of the doorway. The employee exited the scissor lift, started walking, fell, and struck his head on the concrete floor. Employee was admitted to York Hospital.

On or about January 13, 2015, at about 8:05 p.m., a temporary worker was hospitalized for a compound fracture. His arm got caught in and crushed by the rubber bucket mill rollers after his sleeve became caught in the machinery. He has had surgery for the injuries sustained. He was employed by Aerotek, Inc. and placed at the Trelleborg Wheel Systems dba Maine Industrial Tire LLC Red Lion worksite.
